Prognostic Effect of the Nocturnal Blood Pressure Fall in Hypertensive Patient.

บทคัดย่อ
การพยากรณ์โรคที่สำคัญของการตกลงของความดันซิสโตลิกในเวลากลางคืน (nocturnal systolic blood pressure, SBP) ในระยะเวลา 24 ชั่วโมงนั้นยังไม่มีข้อมูลชัดเจน. การศึกษา The Ambulatory Blood Pressure Collaboration in Patients With Hypertension (ABC-H) ได้ทำการวิเคราะห์แบบ meta-analysis ในผู้ป่วยจำนวน 17,312 คน. โดยปัจจัยเสี่ยงได้ถูกคำนวณ สำหรับ the systolic night-to-day ratio และสำหรับ dipping pattern ที่แตกต่างกัน (extreme , reduced และ reverse dippers) กับ normal dippers.

ผู้ทำการศึกษา ABC-H ได้ทำการวิเคราะห์ multivariate adjusted hazard ratios (HRs) ทั้งที่ ร่วมกับ และไม่ร่วมกับ การ adjust 24-hour SBP สำหรับ total cardiovascular events (CVEs) , coronary events , strokes , cardiovascular mortality และ total mortality.

ภายหลังจากการทำ adjust 24-hour SBP พบว่า systolic night-to-day ratio มีความสัมพันธ์กับการพยากรณ์สำหรับผลทั้งหมด โดยการเพิ่มขึ้น 1-SD พบว่า summary HRs เท่ากับ 1.12 ถึง 1.23.

สรุป จากการศึกษา meta-analysis ที่มีขนาดใหญ่ที่สุดนี้ พบว่า การลดลงของ nocturnal BP นั้นมีผลต่อการพยากรณ์ของโรค โดยไม่พบความสัมพันธ์กับค่าความดันซิสโตลิกที่ 24 ชั่วโมง.
